Rhinoplasty 9

This is a 35-year-old female who is very healthy.  She is 5 feet 9 inches and 148 pounds.  She is a 34 B cup, she has bilateral pseudoptotic B cup breasts.  During the original surgery consultation, she was sized with 450 cc implants, but on the morning of surgery, she has stated that she wants to go bigger between 450 cc and 500 cc

This is a 42-year-old female having had two children and breastfeeding both.  She has currently 34 large B, small C cup breasts.She is 5 feet 7 inches and 140 pounds.  Bilateral breasts are pseudoptotic large B, small C cup, nipple-to-mid-clavicle distance of 23 cm and 22 cm, and no palpable masses. During the physical examination, the patient was sized, and she wanted to be small D cup.  We sized her with 450 cc implant, and it looked good on her and it completed her contour.

This is a 31-year-old female who is 5 feet and 6 inches and 145 pounds.  On physical examination., she has small B cup breasts.  The right breast is slightly larger than the left breast by 20 to 25 cc.  Otherwise, she has very good shape to her breasts.  They are teardrop shaped, and the patient has been sized originally with 325 to 350 cc range.

This is a 41-year-old female who has breastfed two children and has 34 large B, small C cup pseudoptotic breasts.  She is 5 feet and 1 inch and 120 pounds. The patient has been sized with 450 to 500 cc implants, she wants to be a solid D cup.

This is a 42-year-old female who is 34 A cup.  She has had four children and breastfed all four.  She is 5 feet 2 inches and has bilateral A cup breasts with grade 1 ptosis and no palpable masses.  She is very muscular.  The patient has been sized with 350 to 375 cc implants.  She wants to be a full C cup just to balance out her body.

30-year-old  Patient was an athletic and lean 5’2”, weighing in at 120 lbs with 32A cup breasts that had deflated due to breastfeeding.

The team had sized her with 400cc implants during the original consultation. However, once they sized her again on the table during surgery, her dominant shoulders swallowed the implants and made them look too small. They therefore upsized her, and at 450cc, they noted that she was in full symmetry, and her shoulders flowed into her hips with the right size transition of the breasts.

She was then augmented with Mentor Smooth Gel 450cc Gummy Bear Implants inserted under the pec muscles. She ended up filling out her breast tissue and at the same time, she did not look “done.”

48-year-old lady who came to Cosmetic Surgery Institute from Orange County.  This patient was a 34A cup at 5” tall and 105 lbs. She had a history of breastfeeding two babies. As this woman was very small and petite, the surgeon had to be careful not to choose overly large implants for fear of making her look top-heavy. She was sized at 350cc, and this was an excellent choice in balancing her shoulders into her hips.

For her breast augmentation at the Los Angeles practice, Dr. Mo Zakhireh augmented the patient with Mentor Smooth Gel 350cc Gummy Bear implants, which were placed under the pectoral muscles for mammogram reasons.

IL was a 20-year-old female with a 34A cup breast size at 5'5" and weighing 128 lbs. This patient was in excellent shape with a very low body fat percentage and had just returned from a bodybuilding competition. She related that she had been eating healthily and normally and was not planning on doing more competitions anytime soon. She requested 500cc implants, but Dr. Mo Zakhireh advised the patient that while he could do this size for her, going from an

A cup to a D cup all at once might give her stretch marks.

The patient then asked what Cosmetic Surgery Institute could do for the possible stretch marks if they did happen. Dr. Zakhireh educated the patient about the BBL laser, which could reduce the red in her skin. The patient proceeded with surgery, where she was augmented with Mentor Smooth Gel 500cc High Profile Gummy Bear Implants. These implants were placed under the pectoral muscles for mammogram reasons.

CR was a 25-year-old female who came to Cosmetic Surgery Institute requesting a breast augmentation from the Los Angeles practice. She had no children, was a 34B cup at 5'9", and weighed 130 lbs. The patient was originally sized at 500cc. However, on the morning of surgery, the team resized her, and she decided that 500cc was too big, so she downsized to 450cc, which was a good fit.

In surgery, Dr. Mo Zakhireh augmented her with Mentor Smooth Gel High Profile 450cc gummy bear implants. The surgeon placed them under the pectoral muscles for mammogram-related reasons. The patient did very well and had a very smooth post-op recovery course. She was so happy and very grateful for her results.

Collagen Induction Therapy stimulates hair growth using your body's own platelets. During a collagen induction therapy treatment at Los Angeles-based practice, Cosmetic Surgical Institute, some of the patient’s blood is taken and spun in a centrifuge. This sample of blood, which contains a high concentration of platelets and growth factors to accelerate growth, is reinjected into the scalp. These before and after photos were taken after three treatments performed by Adrienne Bloom, which were spaced one month apart.

This patient complained of thinning hair around her hairline. She didn't like how thin her hair was when she pulled it back or wanted to wear it up. Esthetician Adrienne Bloom from Cosmetic Surgery Institute treated the patient with collagen induction therapy hair restoration injections. The "After" pictures were taken four months after her initial hair collagen induction therapy from the Los Angeles area practice.

This is a 50-year-old gentleman who comes in for otoplasty, and he is otherwise very healthy.  On physical examination, he is 5 feet 7 inches and 156 pounds.  He has distance from the helical rim to the scalp to 3 cm at the superior helical rim, and he has an obtuse and diffusely formed antihelix on both sides.  He has a very prominent and deep conchal bowl, the right side more than the left, and I have advised the patient that he needs both conchal bowl reduction, and he needs antihelix cartilage weakening and formation of the antihelix.

This patient is 6 feet tall and 145 pounds.  He has bilateral prominent ears with very deep inset conchal bowls and also obtuse angled antihelix.  Helical distance to the scalp is 3 cm.

This is a 70-year-old female with a previous history of face and neck lift in 2011.  The patient on physical examination is 5 feet 8 inches and 150 pounds.  She has very thick leathery Fitzpatrick type 1 skin and very ruddy.  The patient has very puffy lower lids and excessive skin of the lower lids and face and neck laxity with jowling and lots of rhytids around the lips.

This is a 65-year-old gentleman who lost 45 pounds during the COVID by eating healthy and now his neck and face sag and he wishes to have a face and neck lift.  The patient on physical examination is 6 feet tall and 175 pounds.  He is Fitzpatrick type 2 skin complexion, very ruddy, lots of sun damage on the face and extensive neck laxity with lipodystrophy component and neck and jowl laxity and also hollow tear troughs and malar area.

Patient came into the practice with face and neck laxity, neck banding, puffy lower eyelids and underlying, hollow tear troughs. She received multiple procedures including a neck lift and facelift.

Dr. Mo Zakhireh performed a very conservative, muscle-tightening neck lift and facelift.  Addressed her puffy lower eyelids by removing the fat pockets. Frafted fat and stem cells to fill the patient’s hollow tear troughs, areas of enlarged pores, and fine lines around the mouth.

Dr. Mo Zakhireh performed surgery for muscle tightening of the face and neck. Fat was also added to her lips, but not with the intention of making them very large. Fat was added to her hollow lower eyelids and temples.

This is a 60-year-old female with face and neck laxity, periocular rhytids, and perioral rhytids coming in for face and neck lift and fat to face and ALMI to face.  I have also recommended Erbium laser to the skin around the eyes to help tighten the periocular skin, but the patient opted not to do the Erbium, only the ALMI and fat to face.

This is a 56-year-old gentleman with no past surgical history. He is 6 feet 1 inch and 174 pounds. His face is very gaunt and skinny. He has lost quite a bit of fat from mid-face and temporal region, and his lower lids are hollowed out with excess skin and bagginess hanging from his festoons.

This patient is a 44-year-old female who has had a history of lower blepharoplasties done 3 to 4 months ago, and now, she feels that her lower lids are very hallow.  The patient on physical examination has moderate layer of lipodystrophy over the love handles and the abdomen.  She also has moderate amount of skin laxity in the upper and lower abdominal region, she has hallowing in her lower lid tear troughs with crepiness of her skin, especially when she smiles.

This is a 68-year-old female who has face and neck laxity and midface fat atrophy.  She would like to have her neck tightened and the bands done and face tightened and fat to face including her lips.

This is a 60-year-old female with face and neck laxity, periocular rhytids, and perioral rhytids coming in for face and neck lift and fat to face and ALMI to face.  I have also recommended Erbium laser to the skin around the eyes to help tighten the periocular skin, but the patient opted not to do the Erbium, only the ALMI and fat to face.

This is a 59-year-old female She does not want eyes done, does not want fat, and does not want any laser to face.

This is a 69-year-old female who had face and neck lift, quad bleph, erbium laser, and fat to face done with us in 2015.  The patient is back for revision of face and neck lift.  Things have loosened up slightly, but they still look good.  The patient on physical examination is 5 feet 6 inches and 140 pounds.  She is a Fitzpatrick skin type 2, and she has neck banding and facial laxity with deep nasolabial folds and jowls.  Her upper and lower lids still look good, and she has well-healed facelift scars

This is a 52-year-old established patient who has had multiple surgeries with us.   Now, the patient has lost quite bit of weight, and she noticed that her face is sagging. This patient is 5 feet 6 inches and 180 pounds.  She has face and neck laxity, heavy upper and lower eyelids, mid-facial fat hollowing and also flat lips, and neck lipodystrophy.  The patient is a Fitzpatrick type 2 skin, and her skin is very, very thin and wrinkly, and she has quite a bit of sun damage. The patient is a nature lover and outdoorsy.  Therefore, I have not offered her any lasering, but we have offered her face and neck lift, upper and lower bleph, fat to full face and lips, and liposuction of neck.
